I have a 2000 TE 28 with Yanmar 6LP-STE. There appears to be an extension between the turbo and mixing elbow which on mine is rusted out.
Does anyone have any information on this and how it can be replaced.

That is called the exhaust bend (aka dry bend). See attached mixing elbow diagram which includes the dry bend and part numbers.

If it is rusted, your elbow is more than likely as well. I had a stainless elbow and dry bend made after mine rusted off the engine upon launching. I think there is a thread on here with my pics(search for 'mixing elbow"). It wasn't much more than Mack Boring wanted for cast iron OE parts that would have to be replaced again. John at Oxford Yacht (410-822-8556) can hook you up with one.
