The water pressure at my transom shower is abysmal. Is this normal or do I need to "get upside down" to investigate.

Depends on which one it is. The one that I have has a all plastic head and a phillips head screw in the center. If you take it all apart there is more than likely grit and gunk in it. That was the problem with mine. I do not rember if there is a screen under the handle or not. JACK
